Providence Health & Services Physical Therapy Assistant Home Health Westchester to Hollywood in Santa Monica, California


Providence is calling a Physical Therapy Assistant (Full time/Day shift) to Providence Little Company Of Mary Home Health based in Torrance, CA.

This position will provide coverage from Westchester to Hollywood.

We are seeking a Physical Therapy Assistant who will assume responsibility and accountability under the supervision of the Physical Therapist for a designated group of patients and provide physical therapy modalities and treatments in accordance with established hospital and departmental policies and procedures, maintaining the highest degree of quality of patient care for individuals with physical impairments. S/he is a licensed professional who is responsible for maintaining standards of professional and ethical practice in accordance with regulatory agencies and the American Physical Therapy Association (APTA). Providing care to; pediatric, adolescent, young, middle and older adult patients utilizing therapeutic modalities and principles in an interdisciplinary collaborative approach.

In this position you will:

  • Provide direct physical therapy interventions based on established treatment plan and goals

  • Demonstrate proficient knowledge and ability to apply therapeutic principles and practice

  • Assure the patient/family rights are respected and maintained

  • Adhere to all regulatory requirements

  • Provide family / caregiver training when appropriate

  • Provide for needs of the patient within the scope of licensure

  • Demonstrate ability to apply physical agents and modalities appropriately and effectively

  • Provide interventions consistent with total rehabilitation program objectives

  • Skillfully and safely progress patients through the levels of treatment within the maximum capabilities of the patient

  • Provide community based therapeutic activities as necessary to facilitate skill generalization

  • Use tactile facilitation and cueing at appropriate key points

  • Evaluate patient response to treatment and adapts treatment plan, under the direction of the supervising physical therapist to meet changing patient needs

  • Evaluate the patient’s response to physical therapy intervention in relation to the stated goals on the treatment plan

  • Documentation is completed within timeframe specified in departmental policy and procedure manual

  • Documentation is legible, organized and professional

  • Demonstrates a working knowledge of the hospital information system and confidentiality policies

  • Maintaining confidentiality of records

  • Perform billing duties accurately and timely

  • Able to document accurately patient charges, so that patient’s chart and billing sheets are the same.

  • Documents and informs Lead P. T. and P.T. Supervisor about all incident reports within 24 hours

  • Establish and maintain a therapeutic relationship with patients; use praise and other reinforcers to encourage appropriate behavior

  • Consult with the supervising physical therapist and other members of the rehabilitation team for input into the effectiveness of the physical therapy intervention

  • Provide input to the licensed physical therapist regarding discharge summary

  • Demonstrate the ability to communicate with patients and families in establishing favorable attitudes and motivation towards the rehabilitation process

  • Develop and maintain effective and professional communication with patients, families, and other members of the rehabilitation team

  • Participate in problem solving conflicts ie overlapping responsibilities, scheduling problems, etc.

  • Participate in regularly scheduled meetings to provide input to the treatment plan and discuss pertinent patient problems and concerns

  • Participate in the orientation of new staff and contract staff

  • Function as a resource to other members of the rehabilitation team


Required qualifications for this position include:

  • Coursework/Training Sufficient for license

  • Requires current California State Physical Therapy Assistant License

  • Current BLS-HCP – Basic Life Support - for Healthcare Providers - American Heart Association (AHA), or American Red Cross (ARC) - CPR/AED for Professional Rescuer accepted on hire until card expiration date

  • Requires good working knowledge of general and specialized clinical equipment

  • Requires ability to perform all physical requirements pertaining to delegated tasks

  • Requires ability to organize and prioritize work, completes assignments on time

  • Requires ability to establish and maintain a courteous and cooperative working relationship with patients, families and coworkers

  • Requires ability to type and use a computer efficiently

  • Requires the ability to work effectively independently and with a team
